Family Health Insurance in Spain: Compare Costs and Cover

Comparing health insurance for several people involves more than looking at a monthly premium or a plan labelled “family”. The insured people’s ages, where they live, the cover they need and the plan selected can all affect an offer. To make a useful comparison, request quotes for the same household members and check the terms for each person.
This guide focuses on how to prepare that commercial comparison. It does not publish rates or name a universal winner: prices, acceptance and terms must be confirmed in the current quote and product documents.
What to prepare before requesting a family quote
Prepare a profile for each person you want to insure. This lets you request comparable quotes and check that every proposal covers all the household members you listed.
- ✓People and ages: list everyone to be insured and confirm each person’s eligibility and applicable terms separately.
- ✓Where you will use the cover: provide the postcode and check that the local medical network includes the doctors, clinics and hospitals you need.
- ✓Cover requirements: specify whether you need consultations, paediatrics, specialists, diagnostic tests, emergency care, hospitalisation or other services; verify what the plan includes and any limits.
- ✓Expected use: estimate which services each household member may use. This is not a medical prediction; it helps compare the premium with possible copayments.
- ✓Each applicant’s circumstances: answer application questions accurately and request written confirmation of any acceptance, exclusion or special term.
How to compare prices, copayments and terms
Request quotes for the same list of people and the same cover. Compare the household’s total cost, not just a price shown for one person or an introductory promotion. If a family discount is offered, check who qualifies, how long it lasts and what the renewal price will be.
| What to compare | What to ask or check |
|---|---|
| Premium and total cost | What is the premium for everyone and the annual cost? What price applies after a promotion? |
| Copayments | Which services have a copayment, how much is each use and are there limits or tiers? |
| Cover | Which services are included for each insured person, and what exclusions or limits apply? |
| Medical network | Are the doctors and facilities you need in your area included, and are they accepting patients under this plan? |
| Waiting periods | Which waiting periods apply to each service and from what date can it be used? |
| Policy and renewal | Will cover be issued under one policy or separate contracts? When does it start and how does it renew? |
A plan with copayments may have a different premium from one without them, so comparing the monthly payment alone is not enough. ASISA lists different health insurance options, and Sanitas explains plans with copayments, without copayments and with reimbursement. Review the exact product terms and calculate reasonable use scenarios for your household; do not assume one option will always cost less.
Check the medical network and services each person will use
Doctors and facilities available to you depend on your location and the product’s medical network. Search for professionals you already use and confirm that they appear in the current network. If a service matters to your family—such as paediatrics or specific diagnostic tests—check that it is included and review its limits, authorisations and waiting periods.
- ✓Households with children: check the paediatric cover, emergency care and preventive services you need in the product terms.
- ✓Frequent medical use: compare the annual cost, including the copayments that may apply to each person.
- ✓Specific doctors or facilities: check the provincial medical network and confirm availability before buying.
- ✓Family members with different ages or needs: ask for a quote that shows each person’s premium and terms as well as the household total.
Steps for comparing family health plans
- ✓1. Decide who needs cover and prepare the same information for each insurer.
- ✓2. Choose comparable cover: do not compare an outpatient-only plan with a plan that includes hospitalisation as if they were equivalent.
- ✓3. Request the full offer: ask for the premium per person and household total, plan type, copayments, covered services, waiting periods, exclusions and renewal terms.
- ✓4. Verify the details in writing: confirm acceptance for every household member and any discount or special term before paying.
- ✓5. Compare the cost for your situation: add the premiums for everyone insured and consider likely copayments without treating an estimate as a guarantee.

Frequently asked questions about family health insurance
- ✓Is there one standard price for family health insurance?: There is no single rate that applies to every household. Request a quote based on your household members, ages and cover, and check how long it is valid.
- ✓Does insuring the whole family always include a discount?: Do not assume so. Ask for the discount, eligible people, duration and renewal price to be shown in the offer.
- ✓Does every family member have the same terms?: Not necessarily. Check each person’s acceptance, cover, copayments, waiting periods and exclusions in the policy documents.
- ✓Which is better: a plan with or without copayments?: It depends on the quoted premium, applicable copayments and expected use by each household member. Compare the total annual cost rather than deciding by the premium alone.
- ✓Does family health insurance automatically meet residence application requirements?: No. Requirements depend on the procedure and personal circumstances. Read our guide to health insurance for family reunification and check the official instructions for your application.
